AMC is partnering with PathFinder Health again this year to offer you the program.

With CheckUps, we’ve made your annual preventive health visit easier than ever.

You can complete all steps from the comfort of home—no driving or crowded waiting rooms needed (when you choose a lab kit). Best of all, there’s no cost to participate.

It is available to all insured associates and any adults aged 18 or older who are covered under the associate's plan. Depending on your benefits, you can earn a monetary incentive for completing a CheckUp. Please note: This will serve as your annual insurance-covered preventive visit. Labs will not be released without the doctor visit.

Potential Incentives from AMC benefits

LiveWell credit for the Cigna HSA Select plan

Associates and covered spouses/domestic partners enrolled in the HSA Select medical plan
Save up to $520 a year ($20 off bi-weekly rate) this year and in 2027 by completing two easy tasks.
- Complete a preventive exam – a exam will satisfy this requirement
- Complete a health assessment through myCigna
The sooner you get your CheckUp, the more money you will save!

Wellness incentive from Voya

If you are enrolled in Voya Accident and/or Critical Illness coverage – simply file a completion claim to receive benefit payment
Receive a payment of $50 for each person on each plan when you complete a covered health screening test.
A exam satisfies this requirement for enrolled associates and spouses/domestic partners

Blood Sample Collection Choice for your Virtual CheckUp

Lab Kit (self-collection at home) / Lab kit will be shipped to your address for sample self-collection. You will be required to provide 4-6 drops of blood by using a simple lancet fingerstick. All necessary materials for successful collection will be enclosed in your kit, including a pre-paid USPS return label. See instructions below or click here for a video tutorial.
