HIPAA / SUMMARY OF PRIVACY PRACTICES / SCOPE OF CARE
The following is a summary of our privacy practices which contains a condensed version of our Notice of Privacy Practices. The complete notice is available upon request for your review. This summary describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
We understand that your medical information is personal to you, and we are committed to protecting the information about you. As our patient, we create medical records about your health, our care for you, and the services and/or items we provide to you as our patient. By law, we are required to make sure that your protected health information is kept private. We reserve the right to revise this summary of our Notice of Privacy Practices at any time. If and when revision occurs, we will post a copy in our offices and/or website and provide you with a copy upon your request.
How will we use or disclose your information? Here are a few examples (for more details, please refer to the full-length Notice of Privacy Practices): for medical care and treatment, to obtain payment for our services, to generate aggregate data, in emergency situations, to avert a serious threat to health or safety, for appointment and patient recall reminders, for quality improvement initiatives and disease management programs, and to respond to lawsuits and legal actions.
Completing a Preventive Medical Physical Exam or a Health Risk Assessment (HRA) may qualify you for an employer sponsored incentive if you are eligible under your company’s current policy. Understand that all of your personal health-related information will be kept strictly confidential as outlined in the Notice of Privacy Practices. The only information shared with your employer is the fact that you have completed an assessment and/or other information required to be eligible for this incentive.
If you believe your privacy rights have been violated, you may file a complaint with Pathfinder or with the Secretary of the Department of Health & Human Services. To file a complaint with Pathfinder, contact an Administrator at 847.686.7284. All complaints must also be submitted in writing. You will not be penalized for filing a complaint. You have certain rights regarding the information we maintain about you. These rights include: the right to inspect and copy your records or receive a paper copy of the Notice of Privacy Practices, the right to amend and an accounting of disclosures, the right to request restrictions, the right to request confidential communications, the right to file a complaint.
PathFinder provides convenient access to preventive health care. The scope of this preventive care is limited to the assessment of risks for important preventable chronic diseases, consultation regarding these risks and formulation of a personal prevention plan intended to address some of these risks. Our care does not include diagnosis or management of acute or chronic health concerns. If a risk is identified, you will be referred to your community health system as the scope of our care is not able to include ongoing health services. We recommend the engagement of all participants with a primary medical doctor in their local health system.

Why should I have a preventive checkup?
Prioritizing preventive care is a smart way to sustain good health. Early detection not only broadens treatment options but can also prevent certain conditions from developing. The result is better health and reduced future out-of pocket healthcare costs.
What is the CheckUps program?
CheckUps is a convenient option to attend a preventive checkup with a doctor, all without stepping foot into a doctor's office. The purpose of the CheckUp is to assess you for important health risks, ensure that you understand your results and to provide you with preventive counseling on ways to promote a healthy lifestyle.
Who can participate?
The CheckUps program is available to all insured Teammates and their covered dependents enrolled in a WellSky-sponsored Meritain medical plan.
What is involved?
*labs will not be released without a discussion with the doctor
What labs are collected and discussed?
A1c (blood sugar) and a complete lipid panel (Total cholesterol, LDL, HDL, triglycerides)
How do I collect my lab sample?
When you register, you will select your preference for lab collection (home lab kit or visiting a local LabCorp). If you select a home kit, you will receive it within a few days and it will includes step-by- step instructions. This option walks your through how to perform a simple finger stick to collect 4-5 droplets of blood on a sample card using a provided lancet (no needles!).
Do I need to fast for my lab collection?
Fasting is completely optional and NOT required.
How much does it cost to participate in CheckUps?
There is no cost to you! However, your CheckUp is considered your annual insurance-covered preventive exam. Any additional follow-up exams with another doctor will be billed separately and may be subject to a copay.
Will my information be shared with anyone?
No. The CheckUps is completely confidential and HIPAA-compliant. Your record will not be shared with your employer or any other third party

¿Cómo recojo mi muestra de laboratorio?
Cuando se registre, seleccionará su preferencia para la recogida de muestras (kit de laboratorio casero o visita a un LabCorp local). Si selecciona un kit casero, lo recibirá en unos días e incluirá instrucciones paso a paso. Esta opción le guiará a través de cómo realizar un simple pinchazo en el dedo para recoger 4-5 gotas de sangre en una tarjeta de muestra utilizando una lanceta proporcionada (¡sin agujas!).
¿Necesito ayunar para las pruebas de laboratorio?
El ayuno es completamente opcional y NO es obligatorio.
¿Cuánto cuesta participar en CheckUps?
No le costará nada. Su CheckUp se considera su examen preventivo anual cubierto por el seguro. Cualquier examen de seguimiento adicional con otro médico se facturará por separado y puede estar sujeto a un copago.
